Glendale, AZ – Fatal Crash Involving Pick-Up Truck and Van at 51st Ave & Glendale Ave

Glendale, AZ (November 17, 2025) – A 70-year-old man died after a violent collision on Sunday, November 16, at the intersection of 51st Avenue and Glendale Avenue. The crash happened around 7 p.m., according to Glendale Police.

Investigators said a Dodge pickup truck drove into the intersection and T-boned a Chrysler van. Jose Ortiz Reyes, who used a wheelchair, was riding in the back of the van when the truck struck the vehicle. Reyes suffered severe injuries and later passed away.

After the initial impact, both the pickup and the van continued moving and hit a stopped Toyota pickup and an ambulance already transporting a patient. Police confirmed that no one in the secondary crash suffered injuries.
