Show Low, AZ (September 15, 2025) – Three people, including two firefighters, were killed in a head-on crash on State Route 260 late Sunday night.
Arizona Department of Public Safety troopers responded around 10:15 p.m. to a collision between a westbound passenger vehicle and a Timber Mesa Fire District ambulance traveling eastbound between Heber-Overgaard and Show Low.
Investigators say the passenger vehicle crossed the center line and struck the ambulance head-on. The crash caused extreme damage to both vehicles.
Both firefighters inside the ambulance and the driver of the passenger vehicle were pronounced dead at the scene. The firefighters have been identified as 31-year-old Brenna Kulikowski and 21-year-old Damon Thompson. Kulikowski had served three years with the department, while Thompson was days away from completing his first year.
Timber Mesa Fire Chief Randy Chevalier described the crash as “horrific” and praised the firefighters’ dedication and skill. Kulikowski was recognized as a compassionate paramedic, and Thompson was remembered as a determined young firefighter who always wanted to serve.
The firefighters were returning to Show Low after transporting a patient to a Valley facility when the collision occurred. The name of the passenger vehicle driver has not been released. DPS continues to investigate the cause of the crash.
Governor Katie Hobbs released a statement expressing sorrow over the loss of the two firefighters and offering condolences to their families and colleagues. The White Mountain Firefighters Association and GoFundMe campaigns have been established to support the families.
We extend our deepest sympathies to the families, colleagues, and communities affected by this tragic incident.
Head-On Accidents in Eastern Arizona
Head-on collisions are among the deadliest types of car accidents in Arizona. These crashes often result in severe injuries or fatalities due to the combined speed and force of impact. Eastern Arizona highways, including SR 260, can be especially hazardous at night, with winding roads, limited lighting, and higher chances of vehicles crossing lanes.
Most head-on collisions are caused by impaired driving, distracted driving, or driver fatigue. In some cases, drivers misjudge curves or lose control, leading to catastrophic accidents. Emergency vehicles, like ambulances, are particularly vulnerable when responding to calls, highlighting the risks first responders face daily.
Victims of these crashes can sustain serious injuries, including traumatic brain injuries, spinal damage, and broken bones. Families often face medical bills, lost income, and emotional trauma following such accidents. A wrongful death attorney can assist mourning families in pursuing compensation for medical costs, lost wages, pain and suffering, and other damages. Legal guidance can be crucial when determining liability and negotiating with insurance companies.
Community awareness and safe driving practices are critical to reducing these tragedies. Drivers should remain focused, avoid distractions, and never drive impaired. Observing speed limits, especially on rural highways, can prevent devastating head-on collisions.
